You can create a contact form from scratch (or from a template) using an online form builder like Jotform. To build a contact form with Jotform from scratch, follow these simple steps: Open the Form Builder. If you’d like, add your organization logo and organization name at the top, then click Save. Enter a title for your form, like “Contact Us.”

To do this, navigate to WPForms » All Forms in your WordPress admin panel. Then, click the ‘Add New’ button. This will open the WPForms drag and drop form builder template library. Start by giving your contact form a name and then select your contact form template. To choose a template simply hover over it and click the ‘Use Template’ button.

To get started, head over to the Google Forms homepage and place the cursor on the multicolored plus sign (+) in the bottom-right corner. The plus sign becomes a purple pencil and purple page icon. Click the purple page icon. Once the window opens, choose the template “Contact information.”
